Q. Where is Central Illinois Bone & Joint Center located?
A. Central Illinois Bone and Joint Center is located at 2905 N. Main St, Suite G, Decatur, IL 62526.
Q. What are your office hours?
A. Regular office hours are Monday – Thursday 7:30 a.m. to 5:00 p.m. and Friday 7:30 a.m. to 4:00 p.m. We are closed one hour for lunch.
Q. At which hospital does Dr. Kefalas Perform Surgery?
A. Dr. Kefalas has privileges at St. Mary’s Hospital, Decatur Memorial Hospital, Dr. John Warner Hospital, and Pana Community Hospital.
Q. What information will I need to bring to my initial visit?
A. At the time of your initial appointment, we ask that you bring the following items: Photo ID, insurance cards, medication list, and all pertinent medical records from your family physician. In order for you to be seen in a more timely manner, we ask that you print off and complete all forms (see link to office forms) and bring them completed with you to your appointment if possible.
Q. Do I need a referral to schedule an appointment?
A. Dr. Kefalas does not require a referral to schedule an appointment. However, in some cases an insurance company will require a referral by your family physician. Please consult your insurance plan for additional information.
Q. Which Insurance plans do you accept?
A. Dr. Kefalas is a preferred provider with most insurance plans. If you have any questions regarding our participation with your plan, please call the billing office at (217) 425-2600.
Q. Will I be required to pay my co-pay at the time of service?
A. Yes, we ask that yo pay your co-pay at the time of service.
Q. What is your cancellation policy?
A. We kindly ask that if you are unable to keep your scheduled appointment that you call our office a minimum of 48 hours in advance.
